This form should be returned to the Agricultural Compliance Team, A207 - 20159 88 Avenue E, Langley, BC V1M 0A4 or an Employment Standards Branch Office near you, together with a fee of $150.00 in the form of a cheque, bank draft or money order payable to the Director of Employment Standards.
